Pharmacological action

Metamizole sodium - the active substance of Analgin - has analgesic, antipyretic and mild anti-inflammatory effects.

The analgesic effect is determined by the suppression of the biosynthesis of endogenous (pain-causing) substances, such as endopraxids, bradykinins, etc. The antipyretic effect is explained by the suppression of the formation and release of substances from bacterial endotoxins, neutrophilic leukocytes, etc.

The action develops after 20–40 minutes and reaches a maximum after 2 hours.

Indications

Mild or moderate pain (headache, migraine, toothache, neuralgia, myalgia, dysmenorrhea, postoperative pain in combination with antispasmodics for renal and biliary colic). Fever in infectious and inflammatory diseases. Use as an antipyretic is advisable after the diagnosis of the disease.

Contraindications

Hypersensitivity to pyrazolone derivatives (butadione, tribuzon), tendency to bronchospasm. Severe hepatic or renal impairment, glucose-6-phosphate dehydrogenase deficiency, blood diseases.

Special instructions

Use with caution in pregnancy (especially in the first trimester and in the last 6 weeks).

In the treatment of children under 5 years of age and patients receiving cytostatic drugs, Metamizole sodium should be administered only under medical supervision.

Intolerance is very rare, but the risk of anaphylactic shock after intravenous administration of the drug is relatively higher than after taking the drug inside.

Patients with atopic asthma and hay fever have an increased risk of allergic reactions.

Dosage and administration

Intramuscularly or intravenously administered to adults, 250-500 mg 3 times a day. The maximum single dose is 1 g, daily - 2 g. Children are prescribed 5-10 mg / kg (0.1-0.2 ml of a 50% solution per 10 kg of body weight) 2-3 times a day.

For children under 1 year of age, the drug is administered only in oil. The injectable solution for injection should have a body temperature. Doses of more than 1 g should be administered iv. Conditions for antishock therapy are necessary. The most common cause of a sharp decrease in blood pressure is an injection rate that is too high, and therefore iv can be administered slowly (at a rate of not more than 1 ml / min), in the patient’s position while lying down, under the control of blood pressure, heart rate and respiratory rate.

Side effects of

In therapeutic doses, the drug is usually well tolerated.

When using Analgin tablets in some patients, adverse reactions may occur: allergic reactions (skin rash, rarely Quincke's edema - anaphylactic shock)

with prolonged use may cause leukopenia, rarely agranulocytosis.

Drug interactions

Tricyclic antidepressants, oral contraceptives, allopurinol disrupt the metabolism of analgin in the liver and increase its toxicity.

Barbiturates, phenylbutazone and other inducers of microsomal liver enzymes weaken the effect of analgin. The simultaneous use of analgin with cyclosporine reduces the level of the latter in the blood.

Sedatives and tranquilizers enhance the analgesic effect of analgin.

Overdose

Symptoms: nausea, vomiting, epigastric pain, vestibular disturbances, tinnitus, clonic and tonic cramps, coma, agranulocytosis, aplastic anemia, hemorrhagic diathesis, skin rash, cardi rash, manifestations.

Treatment: gastric lavage, standard detoxifying measures, symptomatic therapy if necessary.
